We kindly ask that you do not e-mail any medical information (diagnosis, treatments, or lab results) or personal identifying information (full name, date of birth, social security number or insurance information) to our office.
We also ask that if you have specific medical questions or concerns to please make an appointment, as we are unable to answer them over the phone or through e-mail.
